Announcements from the Honey Creek Athletic Department
All students interested in joining a sport at HCMS must complete an IHSAA Physical Form each school year and sign all required forms on their Final Forms account. This program enables all extracurricular permission forms to be completed online by parents and students.
Physical Examination forms (page 3 of the IHSAA Physical) must be uploaded onto the student's Final Forms account. A physical form must be completed prior to participating in conditioning or tryouts. Please refer to the links below.

- Fall Sports: Boys Tennis, Girls Volleyball, Boys Cross Country, Girls Cross Country
- Winter Sports: Boys Basketball, Girls Basketball, Coed Wrestling, Cheer, Dance
- Spring Sports: Coed Swimming, Girls Tennis, Girls Track & Field, Boys Track & Field
